Hello All - 

We are looking to add a question onto our homepage for employees to answer. We want to be able track the answer as we are hoping the answers will give us insight into what types of upcoming labs they would like us to host. 

I am looking for suggestions on how to best execute this.

@dhorne - for your consideration - you could use a third party - like www.jotform.com via the HTML widget.

Jotform supports all types of embeds, iframes, etc to embed on your site seamlessly.

Jotform is one of the better web form builders out there and it supports alot more complexity than surveymonkey.

Agree with @dklinger. You might also consider Cognito Forms or anything here: https://alternativeto.net/category/business-and-commerce/survey-creator/

Thank you for the help - we are trying out Microsoft forms and so fare they have been really beneficial.
