We have a course that has been running well for quite some time. It was created in Articulate Rise, has knowledge checks but no final assessment, and is set to track using 100% course completion. Published in xAPI.

Today, we received some screen shots from a user who completed the course, was given the Exit link, and the received a screen saying “You have failed this lesson.” 

We have never had anything set to show “You have failed this course.” It’s supposed to Pass/Incomplete, but I’m not seeing anywhere to set that. 

The course is also set to infinite tries, and we can’t get back into the course. “Back to the lesson” results in a blank screen.

Any ideas/suggestions?
