After searching more deeply I realise my question is confusing as there is a way to create comments after questions but what i want is a comment section for the user to take notes and give feed after each questions.

hmm if I understand you correctly, sounds like a formal test is not your best option. Now you could add a free text question after each formal question in your test but all that would need to be reviewed in order to generate a final text score. Have you looked at using the Assignment learning object? That might give your users a little more flexibility as they can download the test and upload a completed copy for you to grade. Or perhaps even a Survey could work (has an option to add a comments section to each question but has no score)...depends on what you need this course to do/track, etc.
